In an effort to ensure the continued safety of your children and make sure that they gain the most from our programming, we request that you read the following guidelines and complete the form below.

By participating in Beth Jacob Congregation Youth Groups, you agree to the following guidelines:

• A parent/guardian must remain on the Beth Jacob Campus at all times while your child is in groups.
• The Director and group leaders must be notified of any allergies or medical needs.
• If your child has any special needs, please let us know and we will do our best to accommodate them.
• If a child has a cough or cold, or has had a fever in the last 24-hrs, he/she may not participate in groups. We look forward to seeing your children when they feel better.
• We require your children to have up-to-date vaccinations in order to participate in groups.
• Our youth groups are nut-sensitive. We cannot guarantee that we are nut-free, but we make a sincere effort to.
• Serve only nut-free snacks. Please refrain from sending nut products with your child to shul.
• Children should be dropped off with a counselor at groups and should be picked up at the end of davening.
